Partnership Housing

providing supported accommodation that promotes independence for people with mental health needs.

 What will Partnership Housing do for me? 

The service will support you to become more independent. We will help you build the skills and confidence you need to live in your own home. The service gives you to the opportunity to live in a more independent environment but with 24 hour support or tenancy on call.

 Is Partnership Housing for me? 

If you are over 18 and:

• are experiencing mental ill health
• are already receiving support from mental health services
• have never lived in your own home or haven’t done for a long time
• need support with daily living skills
• need support in managing your home or tenancy

 

 How do I apply? 

Speak to your CPN or Social Worker, they will complete a referral form and send it in on your behalf. After speaking to your referrer, if it looks like we are the right service for you the team will arrange to meet with you.

We can tell you more about the service and give you the chance to look around before you make a decision.

A CPA meeting will then be held where you and all the people involved in supporting you can make sure that everything is in place from the day you move in.

 How does it work? 

You will have your own tenancy support worker who will help make sure that you live in a safe, supportive environment to support you to achieve your goals. With their help you will design your own action plan based on what you want to achieve.
