This
is a partnership between Brighter Futures, Stoke on Trent City Council
and Health and Social Care funding. The team works with people who are
currently rough sleeping across Stoke-on-Trent and Newcastle-under-Lyme.
They provide practical advice and assistance to stop people having to
sleep rough. This help and support includes direct referrals into the
3 main hostels within the local area, access to and using physical and
mental health services, substance misuse services and young people’s
services. The team work closely with local policing units, hostels,
local GP’s and primary health services, mental health resource
centres and services, social services adult and mental health teams,
probation service and the local housing advice centre. In addition a
street outreach service is provided that includes evenings and weekends.
This is to maintain contact and support for people whilst rough sleeping.
