Community Housing aims to support and help you in moving towards independence
Community Housing will provide you with a home of your own. We will also provide the support, help and information to give you the skills and knowledge to live successfully in your own home.
We will work with you to make sure that what we offer is what you need. If your needs change, then we will make sure what we offer changes.
Community Housing is for you if you:
When you and your support worker feel you are ready to move into a home of your own they will make a referral to us. We will then arrange to meet you and together we will complete an application form. If the service is right for you we will place you on the waiting list.
We will keep you updated on your application. When a property becomes available we will ask you to view it and have a look around.
After seeing the property you decide if you want to move in.
If the property or location is not suitable you will go back onto the waiting list until another property becomes available.
If you receive benefits, housing benefit will cover most or all of the cost. You will be responsible for paying your own bills.
With your support worker you will design your own personal action plan based on what you want to achieve. This could include:

Support you to learn new skills which will help you to look after yourself and your home. |

Support for you and your home 24 hours a day for tenancy emergencies. |

Motivation and taking responsibility |
helping you to reach your goals and stay focused. |

Meaningful use of time |
help to find education, training and fun activities that interest you and give you a reason to get up in the morning. |

Social networks and relationships |
help to meet new people and reconnect with family and friends. | |
Physical health |
help to find G.P.s, dentists, etc and how to make and get you to appointments. | |
Emotional & mental wellbeing |
support through difficult times and helping you to use other services that can help. | |
Drug and alcohol misuse |
help to use substance misuse services and support to get you through it. | |
Offending |
support with probation or paying fines. |
