Resettlement and recovery is a specific scheme provided under contract
to the city of stoke on trent social services department and working
within the partnership project accommodation scheme which provides supported
housing for people who have mental health needs and have been living
in Hillcrest hostel.
The scheme allows you to have your own licence agreement and to receive
support and advice with housing related issues from Brighter Futures
support team along with continued support from Hillcrest..
The scheme is flexible and is designed around your own support needs, at the point where you no longer need such levels a referral can be made to the partnership project accommodation scheme or any other supported housing provider in the city.
People who may benefit from the service are:
Men and women aged 18+ who are currently recieving support due to enduring
mental health needs and would benefit from assistance with daily living
skills, tenancy management and signposting to other relevant services.
Providing supported accommodation that promotes independance for people with mental health needs.
The aim of the project is to bring together services that support individuals experiencing mental distress, facilitating their recovery towards independance and promoting equality of opportunity.
The multi-agency partnership enables individuals to have a tailor made package of support that brings together the combined specialist skills and knowledge of all involved.
A collaboration between the tenant, brighter futures, stoke-on-trent social services and north staffs combined health care NHS trust.
Shared homes and single flats across a city wide area internally decorated and furnished to a high standard what type of support is available?
A professional involved in your support (for example your social worker or CPN) must complete a referral form. your referral will then be discussed at the next monthly review meeting.
If your referral has been successful you will be invited to an informal meeting to discuss your individual support needs and the type of accommodation available.
You will be kept informed about the progress of your referral at all stages.

Monthly reviews are held between the agencies involved, at the meetings we discuss new referrals and existing peoples needs.

Floating support is a “tenancy support” service to assist people to manager in their own home.
There is no charge for the service it allows you to live or remain in your own tenancy and to receive support, help and advice with housing related issues. It is a service that is highly flexible and is designed around your own support needs.

We can offer the service within the City of Stoke on Trent and we also offer a scheme in the Newcastle area for young people.
This service is a partnership arrangement with the local housing providers. If you are currently an existing tenant, your housing provider may refer you for the support service and you would have to make an application to ourselves.
